(Common)Class Absences Procedure due to Infectious Disease (ex. Influenza)2020年04月23日

In the event that you are unable to attend classes due to an infectious disease such as influenza, please inform your instructors of your absence from classes and ask for due consideration regarding your absence. It is necessary to undergo medical treatment at home while recovering.
Class Absence Procedures
1. Notify the Office of Academic Affairs / GBP・SCOPE office immediately that you have contracted one of the diseases listed below. （Notification from a main guarantor can be accepted.）
Student ID Number, Student’s Name, Infectious Disease Name, Date of Contraction, Infectious Disease Symptoms, Date of Doctor’s Consultation, Medical Facility’s Name, Last School Attendance Day, Having Vaccination or Not, must be notified to the Office of Academic Affairs.
2. Submit the following documents to the Office of Academic Affairs as soon as possible after recovering:
※In the case of late submission, consideration regarding your absence may not be asked for. Please notify the Office of Academic Affairs if some problems occurred.
Seasonal Influenza (Flu) (Except Avian Influenza)
① Doctor’s note
→The period of absence due to an infectious disease must be shown on the Doctor’s note. *1
② The copy of ‘Course Enrollment Notification ’ *2
Infectious Disease Other Than Seasonal Influenza (Flu)
① Doctor’s note
→The period of absence due to an infectious disease must be shown AND certifies that ‘you have recovered from an infectious disease’ / ‘school attendance is allowed’ on the Doctor’s note. *1
② The copy of ‘Course Enrollment Notification’ *2
*1 Please ask a medical institution to write the contents needed and check issuing fee
for Doctor’s note in advance.
*2 ”Course Enrollment Notification” can be obtained from the ‘Information Portal Site’.
3. Please give a “Confirmation Form for Class Absences Due To Infectious Diseases”
which was issued at the Office of Academic Affairs with a stamp to the
instructors whose classes were missed.
Target Infectious Diseases

Influenza, Whooping cough, Measles, Mumps, Rubella, Chicken Pox,
Pharyngoconjunctival Fever, Tuberculosis, Meningococcal Meningitis, Cholera,
Shigellosis, Enterohemorrhagic Escherichia Coil Infection, Typhoid Fever,
Paratyphoid Fever, EKC: Epidemic Keratoconjunctivitis,
Acute Hemorrhagic Conjunctivitis, and Other Infectious Diseases (In the case of
absence required by a medical institution).
